We have investigated a lot of different types of maintenance errors. These have included;

• Damage to aircraft during jacking
• Incorrect fitment of pitch trim actuator
• Pitch trim servo installation
• Fuel nozzle installation error
• Jammed elevator
• Trim runaway
• Incorrect installation of windscreen
• Autopilot installation error
• Incorrect maintenance procedures

During these investigations we found lots of contributing safety factors which included;

• Human factors
• Supervision of personnel
• Approved data errors
• System design
• Equipment related factors
• Facility related factors

We also made many recommendations, some of which related to the original equipment manufacturer.
